Provides the single entry point for people requesting determination of eligibility to access developmental disabilities services in Weld County.
Determination of an Intellectual or Developmental Disability (I/DD) is required prior to accessing any services through the developmental disabilities system in Colorado.
Case Managers work with individuals and families to gather necessary information and to complete intake paperwork for eligibility determination.
