Description: Prenatal/Postnatal Home Visitation (Nurse-Family Partnership) includes: Expectant and new mothers will be connected with a nurse who will visit every week or two during their pregnancy and up until the baby is two years old. Nurses can help mothers: - Have a healthy pregnancy and a healthy baby - Become a stronger parent - Build a strong network of support - Make the home a safe place for the baby to live and play - Get referrals for health care, child care, job training, and other support services available in the community - Find ways to continue their education and develop job skills - Set goals for their family's future and find ways to help reach them.
