Description: The Options for Long Term Care (SEP) program assists : - Elderly and individuals with disabilities in staying in their home as long as possible. This is a Health First Colorado (Medicaid) program in collaboration with the Department of Human Services. This program offers individuals and their families alternatives to nursing home care. - Case managers assess clients to determine if they are functionally eligible for the program and if so, can assist clients in getting help with homemaking, personal care, non-medical transportation, home modifications, life alert buttons, medication machines, and respite care.
