Provides collaborative, personalized Diabetes Self-Management Training

(DSMT) that allows individuals to successfully manage their diabetes. The

program is accredited by the Association of Diabetes Care & Education

Specialists and provides evidenced-based information and education for:

  • Diabetes education
  • Medication and insulin support
  • Continuous glucose monitor training and support
  • Insulin pump support
