Description: Northwest Colorado Options for Long Term Care is a nine county, single entry point agency which offers case management services for persons entering and receiving services from Medicaid long term care programs. Program options for LTC with Medicaid: - Children's Home & Community - Based Service Program (HCBS) - Home Care Allowance Program (HCA) - Home & Community-Based Services for Persons with Brain Injury(HCBS-BI) - Home & Community-Based Services for the Elderly, Blind and Disabled (HCBS-EBD) - Home & Community-Based Services for persons with Mental Illness (HCBS-MI) - Consumer Directed Attendant Support Program (CDAS) - Home & Community-Based Services for Persons Living with AIDS (HCBS-PLWA) - Nursing Facility Care Program options for LTC without Medicaid: - Non-Medicaid Services - Caregiver Support Program - Senior Equipment & Services Program - Aging & Disabled Resources for Colorado (ADRC) In addition, NCOLTC provides support programs through the Area Agency on Aging (AAA).
