Assists adults at high risk of developing type 2 diabetes reduce their risk for developing the disease by taking steps that will improve their overall health and well-being.
The program provides 1 year of support with a CDC-approved curriculum which includes an emphasis on:
Adults who are at high risk for developing diabetes

Provides information and resources through a team of Promotoras (cultural and linguistic liaisons) who advocate on behalf of the community and build trust between community members and local service agencies.
