Description: Provides assistance to pregnant women and families with young children access health care and find a medical home. Services provided: - Access to health insurance programs Medicaid and CHP+ - Prenatal Care- Help with making doctor appointments - Information on nutrition, growth and development - Healthy lifestyles education - Baby & Me Tobacco Free Program - Translation services - Transportation to and from appointments Staff assists with paperwork and if possible a medical card issued the same day.
