Offers a variety of programs to assist consumers in achieving independence, including the following services:

• Independent living Services that provide skills training, assistance, and resources to people with disabilities to support independence
• Employment program that offers comprehensive job training and support
• Youth transition services for youth aged 14 - 24 years old in preparation for transition from high school to adulthood
• Nursing home transition program that transition adults with disabilities from nursing care to independent living
• Beyond Vision Program, which provides support services, community outreach, and facilitates support groups for individuals who are blind or visually impaired

Application Process: Call for more information or to discuss details of a program or service. If a program or service is appropriate, an appointment will be made with an Independent Living Advisor who will start the intake process. During the intake, goals will be established and client information will be gathered.
