Application Process: Call and register for services. In Adams, Arapahoe, Denver, or Douglas counties, call Rocky Mountain Human Services at (844) 790-RMHS (7647). For Boulder or Broomfield counties, call Adult Care Management, Inc. at (303) 439-7011. The Single Point of Entry (SEP) will do an intake and schedule an assessment of personal care needs with their case manager at client's home, and refer client to the personal care agency to start personal care services. Call for more information and to schedule an interview to discuss services with ILRCCO.
