Helps households experiencing homelessness complete the community housing assessment, known as the Vulnerability Index - Service Prioritization and Decision Assistance Tool (VI-SPDAT). Households are prioritized and ranked based on vulnerability. More vulnerable households are matched to available and appropriate housing programs on an ongoing basis.
While individuals are on the list, the agency that filled out their assessment will reach out to them on a monthly basis to update their whereabouts, contact information, etc. Because of this, each agency can only support a limited number of individuals. Individuals are encouraged to complete their VI at a location they already stop by on a frequent basis.
