Application Process: Call and leave a message to receive a Request for Funds form. The completed form must be mailed to the mailing address. Board meets once a month to discuss requests, and the person making the request may be invited to the board meeting to answer questions or clarify any information. Recipients of financial assistance will be contacted either by phone or mail.
