Description: Provides financial assistance to elderly and disabled individuals who are receiving SSI or are eligible to receive SSI. Assistance paid directly to eligible individuals as a cash payment (EBT card), who are then responsible for paying the provider of their choice. Services may include non-medical care for daily living activities, chore services, respite care and other types of caregiver support.

Application Process: Apply online at Colorado PEAK or walk in to a Human Services office in Aurora or Littleton to apply online at a free computer kiosk, or print an application and fax, mail or deliver it to either the Aurora or Littleton office.
